I hereby confirm that I have obtained the customer's confirmation that he/she has received, read, understood and agreed to be bound by the Privacy Notice and the customer’s consent for UOB to collect, retain, use, and disclose their personal information for marketing, promotional, and/or cross-selling purposes.

The customer has agreed to the following:

1. To provide UOB with the necessary information; and
2. To allow UOB to collect, use, and disclose their information to relevant third-party solution vendors, as determined by UOB, for the purposes of:
   1. Contacting the customer via telephone and/or email regarding solutions (including third-party solutions);
   2. Providing the customer with the above solutions (including third-party solutions); and
   3. Complying with any regulatory requirements as needed.
